Requirements

Board Certified OB/GYN (Board Eligible not accepted)
Active Michigan Medical License
Michigan CSR at time of submission
BLS Certification
DEA (verbal confirmation at name clear)
NPDB Self-Query within the last 30 days
Clean malpractice and background for quick credentialing
Must be a local provider
