Home Health Speech Language Pathologist

CenterWell Home Health is seeking a PRN Home Health Speech Language Pathologist to provide in‑home speech therapy to seniors and adults with chronic conditions. In this role, you will assess speech, language, cognitive, and swallowing disorders; develop and implement individualized treatment plans; and educate patients and caregivers on strategies to improve communication and safety. You’ll collaborate closely with an interdisciplinary care team, document care in accordance with regulatory and payer requirements, and support patients in achieving functional goals that help them remain independent at home while contributing to a compassionate, patient‑first culture.
