We are seeking an experienced Public Health Program Director to lead the expansion of key rural health transformation and care coordination initiatives, including the rollout of maternal/infant care programs and the procurement and integration of call center and case management software systems.
This role provides strategic oversight, manages grant-funded contracts, coordinates software vendors and internal technical teams, and supports community partners working to improve health outcomes in designated rural areas.
Key Responsibilities
Program Oversight & Leadership: Provide strategic leadership and oversight for enhanced care coordination and rural community health initiatives across designated counties.
Contract & Grant Management: Oversee and monitor grant-funded contracts, including reviewing scopes of work, budgets, expenditures, and deliverables to ensure state and federal compliance.
Vendor & Technical Integration: Serve as a bridge between technical teams and software vendors; work closely with application coordinators during the procurement, development, and implementation of new call center and case management platforms.
Stakeholder & Grantee Support: Act as the primary liaison to funded grantees and community partners, offering technical assistance and guidance to drive successful program execution.
Data Monitoring & Evaluation: Collaborate with epidemiologists and analytical staff to design evaluation frameworks, monitor performance metrics, and review quarterly progress data.
Sustainability Planning: Assist in developing long-term strategies and identifying funding opportunities to sustain program impacts beyond the initial grant cycle.
Fieldwork & Engagement: Conduct site visits, facilitate working sessions, and represent the initiative in regional and statewide partner meetings.
Basic Qualifications & Requirements
Education: Bachelors degree from an accredited college or university in Public Health or a related field.
Residency Requirement: Candidate must be a resident of Tennessee prior to the start date.
Work Arrangement: Remote-capable within Tennessee, with required periodic in-person meetings in Nashville and travel to assigned rural locations.
Location: Remote / Hybrid (Must be based in Tennessee)
Work Schedule: Monday Friday, 8:00 AM 4:30 PM CST (37.5 hours/week)
Travel Requirements: Periodic travel within assigned regions/statewide, plus occasional in-person meetings at the Nashville central office (including Day 1 onboarding and laptop setup).
