- MD or DO
- Board Certified in Obstetrics & Gynecology
- Active Arizona license or IMLC eligibility
- Minimum 1 year of post-residency clinical experience
- NPDB Self-Query required
- Prescreen questionnaire required
- Active Arizona license preferred
- IMLC candidates accepted
- Standard credentialing process
