Start Date: July 28, 2026 (onboarding July 27)

Schedule: Tues-Fri for 6 months

Shift time: 0800 - 1630

Setting: Ambulatory Clinic

Duties: Oversee treatments, New consults, follow ups, planning, status checks, tumor conference participation

Required Procedures: SBRT, IMRT, 3D, SRS

EMR: Epic

Support Staff: 1 RN, 4 Radiation Therapists, 2 medical physicists, and 3 Dosimetrists

Reason for Coverage: Provider resignation

Credentialing Information:

Timeframe: can emergency credential in a week once a completed application is returned

Requirements:

Credentialing with other facilities? No

Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less ? No

• ACLS - Nice to Have
• BLS - Preferred Must have by start date.
• PALS - Nice to Have
• DEA - Preferred Must have for assignment but can do a prescription plan if not issued by start date
• NRP - Nice to Have

Clean Background/Malpractice Highly Preferred Can consider minor/closed cases on a case-by-case scenario with NPDB.
